Incidence: a direct inguinal hernia occurs mostly in adults, males or females, but males are 10 times more likely to get a direct inguinal hernia.

Risk factors: include chronic overweight or obesity, constipation, chronic cough, family history and prior episodes of direct inguinal hernias.
The theory overweight factor on the other hand seems to be contradictory since a study in northern Europe concluded that people with more body fat had less inguinal hernias than people that had a normal constitution. The fact that in case of sudden lose of weight the hernia might slip out 'seems' to be more comprehensible (weakened wall).

Why do direct hernias occur in the middle-aged and elderly?
Abdominal walls weaken with age.
